Laurel Ridge notified the state of nearly 650 job cuts, which are expected to take effect in late June.
The sweeping reductions raise concerns about the future of the facility, which has faced significant challenges in recent months. Among them is the termination of its contract to provide Medicare and Medicaid services.
State inspectors cited Laurel Ridge for multiple violations last year and issued another citation this year.
